The conflict of interest form should be carefully filled out by select individuals.

 

Who should complete the form?

For Volunteers and Others:
For volunteers, editors and others, involved in making procurement decisions or other activities that could represent a potential conflict of interest, the questionnaire should be completed and returned to the IEEE Internal Audit Department for preliminary review.  

For Staff:
This questionnaire is to be completed by all employees with signing authority and forwarded to the Internal Audit Department.

All IEEE Staff are required to take the self training class DL058 to understand the Principle of Business Conduct/Conflict of Interest requirement.

Submitting affirmative answers

Due to the size and scope of the Institute, it is not unusual for an employee, volunteer, editor or others to make some affirmative responses to the questions.  If a question is answered “yes,” it does not mean that a conflict of interest necessarily exists.

All “yes” answers will be reviewed by IEEE Internal Audit Department in the context of the employee, volunteer or other individual’s  respective IEEE assignment(s).

To make an anonymous report of inappropriate financial activities involving IEEE business or compliance with IEEE principles of business conduct, file a report at Ethics Point.
