The Senior member and Fellow grade members evaluating Senior member applications rely heavily on the endorsement and comments provided by references. Please take this into consideration when submitting a reference form. It is helpful if you know the applicant personally or professionally, but this is not a strict requirement. You may request the applicant provide you with their résumé or CV on which to base your recommendation. Providing specific details and dates will be helpful during the peer review process.
NOTE: Effective 16 January 2012, the Word and PDF version of the Senior member application and Senior member reference form is available by request only. Send an email request to senior-member@ieee.org.

Nomination process
Submitting the reference form does not by itself constitute a nomination in conjunction with the Nominate a Senior Member Initiative. In order for the section or society to qualify for the Nominate a Senior member Initiative rebate of US$10 per successful nomination, the nominating section of the Senior member application form must be filled in completely including the name of the nominating entity.
Reference forms must be submitted 10 days prior to an A & A Panel Meeting. Submissions received after this deadline are not guaranteed to be included in an upcoming meeting.
