Instructions:

• Final reports must be accompanied by a Financial Statement showing details of the actual expenditure of funds compared with the Project Budget as originally proposed in the Grant Application. The Financial Statement must be provided on the spreadsheet provided below.   To complete and submit electronically, please view this spreadsheet, use the "File" and "Save As" tabs on your browser to save a copy on your local computer, then insert all details as required, save again, and then send via e-mail as an attachment to foundation-office@ieee.org
• If you are unable to send your reports using the linked web forms, you can use Word to prepare your document and submit the file on a disk or CD and mail it to the IEEE Foundation.   However, be advised that you must provide exactly the same information requested in the web form, using the same sequence and limitation of number of characters for each item.   Contact information is provided on the page How to Apply for a Grant.
