AWARD DESCRIPTION
Recognizes organizations employing IEEE members for contributions to employee continuing education and professional development. The award consists of a brass and walnut plaque.

PLEASE NOTE: Current members of the IEEE EAB, or of the IEEE EAB Awards and Recognition Committee, or members of IEEE staff, may NOT be recipients, nominators or endorsers of EAB Awards.

You have been notified that your name has been submitted as a Reference to support a nomination for the IEEE EAB Society/Council Professional Development Award. With that notification you should have been provided with the following:

  1. A copy of the completed Nomination Form for the candidate that you are requested to endorse
  2. A copy of or link to the Reference Form

REFERENCES ELEIGIBILITY
The Nominator is responsible to provide a minimum of three and a maximum of five presons who have had first-hand experience with the professional development activities of the nominated organization for this award. At least two references must be IEEE members. As noted above, current members of the IEEE EAB, of the IEEE EAB Awards and Recognition Committee, or IEEE Staff may not serve as references for any IEEE EAB award.

PREPARATION OF REFERENCE FORM
The IEEE EAB Awards and Recognitions Committee places substantial reliance on your statements. Statements of reference should cite specific evidence of the uniqueness and impact of the contributions of the candidate. Nonspecific comments, such as this person or work is outstanding, without accompanying detailed justification, are not of value to the IEEE EAB Awards and Recognition Committee. Comments about the relative contributions of any work attributed to more than the nominee, for example, if you are a co-author of a listed activity, are very valuable. All statements of references must be submitted on the 2007 IEEE EAB Employer Professional Development Award Reference Form.

The data on the Nomination Form should not be repeated word for word on this Reference Form. Such appropriation of another's words puts in question the reference's judgment of the candidate’s accomplishments, and, to that extent, diminishes the judges' confidence in the reference and reduces its contribution to the candidate's ranking.

Please complete the form on your computer, save as an MS Word file, and submit as an attached email file to eab-awards@ieee.org, as indicated at the end of the form. ALSO, please email a copy of your completed Reference Form to the Nominator.

All information must be typed in MS Word. Handwritten copy is not permitted.

ADHERE TO SPACE LIMITATIONS FOR COPY
Please confine all text to the space indicated on the form for each item. Addenda are not permitted and will not be reviewed by the IEEE EAB Awards and Recognition Committee.
