These instructions are for Downloadable MS Word Form nominations for the Pre-University Educator Award. Please complete the form(s) and submit via email to eab-awards@ieee.org.

AWARD DESCRIPTION
Recognizes current pre-university classroom teachers who have inspired an appreciation and understanding of mathematics, science and technology and the engineering process in students and have encouraged students to pursue technical careers. The award consists of $1000 and a brass and walnut plaque.

PLEASE NOTE: Current members of the IEEE EAB or of the IEEE EAB Awards and Recognition Committee may not be recipients, nominators, or endorsers of EAB Awards.

Please complete the form on your computer, save as an MS Word file, and submit as an attached email file to eab-awards@ieee.org, as indicated at the end of the form. ALSO, please email a copy of your completed Nomination Form to the Reference Writers you provided. All information must be typed in MS Word. Limit number of words as indicated in the text boxes provided. Handwritten copy is not permitted. Please submit all information in English

CANDIDATES
Candidates for the IEEE EAB Pre-University Educator Award must be current pre-university classroom teachers.

NOMINATOR
The Nominator for this award is required to be a member of IEEE.

The use of the Downloadable Word Nomination Form requires the Nominator to notify References and to provide access to, or copy of, the completed Nomination Form, Reference Form and other information about the process.

The Nominator may direct the Reference Writer to the following link to access the Reference Form. http://www.ieee.org/portal/cms_docs/education/EABAwards/docs/ref_precollege.doc (direct them to the bottom portion of the form).

It is the responsibility of the Nominator to follow up with all References to confirm that all Reference Forms are submitted to IEEE EAB Awards Administration (eab-awards@ieee.org) by 30 April 2006. EAB Staff will be unable to supply any information that is missing from the candidate’s nomination package.

DEADLINE
It is the nominator’s responsibility to meet the Deadline of 30 April 2007.

All nomination materials, including the Nomination Form and all Reference Forms, must be received by IEEE EAB Awards Administration (eab-awards@ieee.org) by the deadline.

REFERENCES
The nominator is responsible for providing a minimum of three and a maximum of five persons to serve as references for this award. One reference must be a supervisor, such as a school principal. As noted above, current members of the IEEE EAB or of the IEEE EAB Awards and Recognition Committee may not serve as references for any IEEE EAB award. It is the Nominator's responsibility to follow up with the references to confirm that all reference forms are submitted to EAB Awards Administration by the deadline. EAB Staff will be unable to supply any information that is missing from the candidate's nomination package.

ADHERE TO SPACE LIMITATIONS FOR COPY
Please confine all text to the space indicated on the form for each item. Addenda are not permitted and will not be reviewed by the IEEE EAB Awards and Recognition Committee.
