Application and Forms

Below are the forms that are required to be completed to sponosor a course and offer IEEE CEUs/PDHs:

CEU Application - This form is what you need to complete to apply to offer IEEE CEUs/PDHs at your workshops, monthly meetings, for tutorials or short courses.

Participant CEU and Evaluation Form - Course organizers should use this form to get feedback on their activity and can submist this form so participants can receive CEUs.

Program Summary - Course organizers submit this form to CEU Administrator using totals from the Program Evaluation Form.

Conference CEU Tracking Form - Offering numerous tutorials at your venue; have participants submit this form for CEU credit.
