INSTRUCTIONS FOR COMPLETING
THE IEEE CONFERENCE PUBLICATION FORM.
* - REQUIRED FIELDS
CONFERENCE INFORMATION
  1. Full Title of Conference. Enter the complete title.
     
  2. Dates & Location of Conference. Spell out or abbreviate the month and use numeric designations for the days and year (e.g., Oct. 8-10, 1996). Enter name of hotel or conference center, city, state or province, and country.
     
  3. Financial Sponsors. Enter the name(s) of all IEEE and non-IEEE financial sponsor(s).
     
  4. Non-financial IEEE Entities. Enter the name(s) of IEEE entities with non-financial involvement.
     
  5. Conference Home Page. If applicable, enter the URL of the Home Page for the conference.
     

 

PUBLICATION INFORMATION
  1. Full Name of Publication. State the full title of the publication as it will appear on the title page. If the conference is part of an ongoing series, it is imperative that the title of the publication remain consistent with those of previous years. Please refer to IEEE Policy and Procedure 6.6 for more information. Contact the IEEE Conference Publications Product Management Group with any questions.
     
  2. Est. Number of Pages. Indicate the estimated number of pages in the conference publication.
     
  3. Paper Size. Indicate the page size of the publication.
     
  4. Est. Number of Papers. Indicate the estimated number of technical papers to be included in the publication.
     
  5. Number of Volumes. Indicate the number of volumes in which the publication will be bound.
     
  6. Binding. Indicate whether the publication will be perfect-bound softcover or hardcover.
     
  7. * Est. Print Run. Indicate the estimated number of copies which are to be printed for the conference and for other (non-IEEE Conference Publications) usage. **NOTE: Please do not fill in the Conference Publications quantity. This quantity will be determined by the IEEE Conference Publications Product Management Group.
     
  8. Copyright. Check the appropriate space concerning ownership of copyright. If the copyright is held by an organization other than IEEE, please provide the name of that organization. Information on copyright procedures and copies of the IEEE Copyright Form are provided in the IEEE Online Resource Center for Conference Publications Organizers, available at http://www.ieee.org/pubs/confpubcenter/index.xml. For any conference publication to which IEEE holds the copyright, it is incumbent upon the Publications Chair to provide each prospective author with an IEEE Copyright Form and appropriate instructions for its completion. Individual papers should not be published in the conference proceedings unless the required IEEE Copyright Form has been returned to the Publications Chair. Copyright forms should be collected and forwarded in one batch with a copy of the table of contents to: IEEE Conference Publications Product Management Group, 445 Hoes Lane, Piscataway, NJ 08854, USA.
     
  9. Est. Delivery Date to IEEE. The target date for shipping the proceedings to the IEEE Conference Publications Product Management Group should be indicated in the space provided. PDF files should be delivered at the same time as the print copies. The conference should attempt to have the proceedings delivered to IEEE as soon after the publication date as possible. Shipping instructions will be included with the Letter of Acquisition.
     
  10. Printer/PDF Vendor. Indicate the name of the vendor who will be printing the publication and producing the PDF files. Please visit http://www.ieee.org/pubs/confstandards/index.xml for information on PDF file preparation.
     
  11. * Electronic Distribution. Indicate whether the publication will be made available electronically. For more information, please refer to the IEEE Conference Publications Policy Statement on Conference Proceedings Disseminated Electronically, available at the Online Resourse Center at http://www.ieee.org/pubs/confpubcenter/index.xml .
     

 

PAYMENT INFORMATION
  1. * Payee. For payment by check, please provide the EXACT name of the entity to whom the payment should be made. For payment via wire transfer, please provide the exact account name. ***NOTE: If payment is to be made to an entity other than the conference written approval must be provided by the Conference Chair and/or Treasurer.
     
  2. * Payment Method. Indicate payment method to be used (check, wire transfer, IEEE Concentration Banking transfer, et al.).
     
  3. * Payment information. For payment by check, provide name and address to which the check should be sent. For payment via wire transfer, provide account number, bank name and address, and SWIFT code. It is the responsibility of the conference to notify IEEE Conference Publications Product Management Group of any changes to the payment/contact information. Any requests for payment method changes that are received after the payment has been processed will be subject to a 10% reprocessing fee. Questions or updates may be emailed to confpubs@ieee.org.
     
  4. Publication Chair. Provide your name, street address, phone, fax, and e-mail address.
     
  5. Date. Indicate the date on which the form was completed.
     
  6. * Submitted by. Please be sure to include the email address of the person submitting the form.
