Pre-University Educator Award - Nomination Form and Instructions

Any person, including a non-IEEE Member, is eligible to serve as a nominator of this award, with the following exceptions: current members of the IEEE EAB or of the IEEE EAB Awards and Recognition Committee are not eligible.

The use of the downloadable Nomination Form, requires the Nominator to notify References and to provide access to, or a copy of, the completed Nomination Form, Reference Form, and other information regarding the process.

It is the responsibility of the Nominator to follow up with all References to confirm that all Reference Forms are submitted to the IEEE EAB Awards Administration Staff by the deadline. EAB Staff will be unable to supply any information that is missing from the candidate’s nomination package. 

The Nominator is responsible for providing a minimum of three and a maximum of five persons to serve as References for this award.  At least two References must be IEEE members. As noted above, current members of the IEEE EAB or of the IEEE EAB Awards and Recognition Committee may not serve as References for any IEEE EAB award.

It is the Reference Writers responsibility to submit the completed Reference Form by the deadline. 

Please confine all text to the space and word limits indicated on the form. Addenda are not permitted and will not be reviewed by the IEEE EAB Awards and Recognition Committee. 

It is the Nominator’s responsibility to meet the deadline of 30 April 2008

All nomination materials, including the Nomination Form and all Reference Forms, must be received by the IEEE EAB Awards Administration staff by the appropriate deadline. 

Please Note: Current members of the IEEE EAB or of the IEEE EAB Awards and Recognition Committee may not be recipients, nominators, or endorsers of EAB Awards.  

Please complete the form, save it as an MS Word file, and submit. Also, please e-mail a copy of your completed Nomination Form to the Reference Writers you provided.

• Limit number of words as indicated in the text boxes provided. 
• Handwritten copy is not permitted. 
• Please submit all information in English.
