Strict adherence to the procedures is essential; otherwise, a nominee may be placed at a serious disadvantage and possibly even precluded from consideration.
All nomination, reference and endorsement forms must be submitted to the IEEE Fellow Committee by the 01 March deadline.

Instructions All forms that are downloaded must be mailed by regular or express mail to the IEEE Fellow Committee, 445 Hoes Lane, Piscataway, NJ 08854.
