The Senior member and Fellow grade members evaluating Senior member applications rely heavily on the endorsement and comments provided by references. Please take this into consideration when submitting a reference form. It is helpful if you know the applicant personally or professionally, but this is not a strict requirement. You may request the applicant provide you with their résumé or CV on which to base your recommendation. Providing specific details and dates will be helpful during the peer review process.

Submitting the reference form does not by itself constitute a nomination in conjunction with the Nominate a Senior Member Initiative. In order for the section or society to qualify for the Nominate a Senior member Initiative rebate of US$10 per successful nomination, the nominating section of the Senior member application form must be filled in completely including the name of the nominating entity.
Reference forms have to be submitted and received at least 10 days prior for US meeting locations and at least 21 days prior for international meeting locations.
